For health professionals, system administrators and planners, and
anyone who wants to do a better job of helping patients help
themselves.
Description:
In chronic conditions, patient outcomes and utilization depend
primarily on their ability to care for themselves. If
self-management is poor, costs will soar and patients will suffer.
Health care systems can build patients’ self-care capacity or impair
it. Systems and practices often need to change to support
self-management, and the result is greater satisfaction and better
results for staff and patients alike. Learn strategies for effective
self-management support including:
- Building self-confidence (self-efficacy) to make personal
changes
- Including families in all aspects of care
- Communicating more effectively to get on the same page with
patients
- Expanding roles of all staff, using volunteers and other
patients to support self-management
- Allying with communities to promote wellness and support
patients
- Basics of useful system changes to support self-management
